In consideration of being permitted access to and use of the Mindful Adventure Studio, I, my family, heirs, executors, representatives, administrators, and assigns do hereby waive, release and forever discharge Amy Vitt of Mindful Adventure LLC from any and all responsibilities, liabilities and lawsuits, present or future, and causes of action for ordinary negligence, whether foreseeable or unforeseeable, arising out of or related in any manner directly or indirectly to my use of or access to the Mindful Adventure Studio, including but not limited to such claims that may result from any injury, illness, or death, accidental or otherwise, during or arising in any way from my participation in any movement exercise or yoga practice associated with the facility, or my using balance equipment, stairs, blocks, straps, strength equipment, free weights, medicine balls, etc. made available in the facility except where that injury is the result of negligence by Amy Vitt of Mindful Adventure LLC.

I understand that the fitness and strength assessments, movements, postures, and breathing practices provided by Amy Vitt of Mindful Adventure LLC do not provide a diagnosis of disease and are not a substitute for regular medical care and consultation. The undersigned hereby expressly agrees that this Waiver and Release of Liability is intended to be as broad and inclusive as permitted by the laws of Arizona, and that if any portion hereof is invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
